What is titration in pharmacology?

Fixed-dose regimen

Gradual adjustment of dose to achieve desired effect

Titration means starting with a small dose and gradually increasing it, measuring the patient’s response at each step until the desired therapeutic effect is achieved with acceptable safety. This approach recognizes that people respond differently to meds, so you adjust the amount to hit the target benefit without causing intolerable side effects. It relies on careful monitoring, predefined dose steps, and sometimes specific endpoints (like blood pressure or symptom relief) to guide how far to raise the dose.

Compared to a fixed-dose regimen, titration allows doses to be tailored to the individual rather than staying the same for everyone. It’s not about just taking more until you see an effect, which can lead to overshoot and more side effects, and it’s not about making abrupt dose changes, which can cause instability or withdrawal. An example is slowly increasing an antihypertensive dose until blood pressure reaches the target range or side effects limit further increases.
